Non - extraction treatment of class ii division 2 malocclusion with forsus fatigue-resistant device – 2 case report

Class II malocclusion is one of the most frequent problems encountered in orthodontic. The most common characteristic of Class II malocclusion is mandibular retrognathia rather than maxillary protrusion according to McNamara. Non-growing patients with Class II mandibular retrusion are mostly treated with fixed functional appliances. This article represents two case reports of Class II division 2 malocclusion cases treated with Forsus Fatigue Resistant Appliance after initial levelling and alignment. Forsus Fatigue Resistant appliance in particular when used in conjunction with full bonded fixed appliance facilitate the correction of Class II Div 2 malocclusion by skeletal and dental changes.

A retrospective study evaluating the influence of Class III correction appliances on the sagittal pharyngeal airway dimension

The aim of this study was to compare the effects of Class III correction appliances including the Facemask (FM), and the new non-compliance fixed functional appliances such as the Reversed Forsus Fatigue Resistant Device (FRD), as well as the CS-2000 (CS), on the sagittal pharyngeal airway dimension (SPAD). Pre-treatment and post-treatment lateral cephalograms of 45 patients who underwent Class III appliance treatment, using either FM, Reversed FRD, or CS were collected from the files of treated patients. SPAD changes were evaluated in each group, and comparisons were conducted between the three study groups. Additionally, sagittal and vertical skeletal measurements were conducted. The FM, the Reversed FRD, and the CS, were found to generate a significant increase in the SPAD, with the Reversed FRD contributing to the most significant change at the OPAA (116.80 ± 26.36 mm2). All three appliances elicited significant antero-posterior changes in the SNA°, SNB°, and ANB°, also with the greatest intermaxillary change documented with the employment of the Reversed FRD (ANB° = 3.33 ± 0.82°). As for the vertical dimension, the FM, the Reversed FRD, and the CS elicited significant FMA° increases, with the greatest change attributed to the FM (FMA° = 2.32 ± 0.97°). Therefore, the three tested Class III corrective appliances generated significant SPAD, antero-posterior, and vertical changes. However, the Revered FRD showed a superior impact in increasing the SPAD at the OPAA level and in eliciting significant intermaxillary changes.

Non-extraction Treatment of Class Ii Division 1 Malocclusion using the Forsus Fatigue Resistant Device – Case Report

Devices commonly used for correction of Class II malocclusions can be classified as extra-oral (headgear), intra-arch or inter-arch. The intra-arch devices are either removable (Cetlin or sagittal appliance) or fixed appliances (Distal-jet, Jones-jig, etc). Fixed intra-arch appliances often depend on a Nance button for anchorage. Interarch devices, which use the mandibular arch for anchorage, can be removable or fixed. They can pull (Class II elastics) or push (Frankel, Herbst, Japer jumper). The Forsus Fatigue resistant device is an inter-arch push spring that produces 200g of force when fully compressed, the force levels being comparable to heavy Class II elastics. The Forsus FRD has be effective in correcting Class II division 1 malocclusions without the need for patient compliance and considerably reducing chair-side time. The optimum force levels increase patient comfort and provides faster resolution of Class II skeletal base maxillo-mandibular relationship.

Treatment of an adolescent patient with class ii division 1 malocclusion by extraction and forsus fatigue resistant device: Case report

Treatment of a patient presenting with Class II malocclusion requires careful diagnosis and treatment planning to resolve esthetic, occlusal, and functional problems. Over the decades numerous treatment options have been presented to treat class II malocclusions. Orthodontic camouflage being a cost-effective, non-surgical treatment option has been well accepted by patients to mask their underlying skeletal discrepancy, while providing acceptable facial esthetics and occlusion.This case report presents the treatment by four first premolar extractions followed by Forsus Fatigue Resistant Device (FFRD) therapy in a 14 years old female patient with skeletal Class II base due to the retrognathic mandible, proclined upper and lower front teeth with an overjet of 5 mm, and unwilling for surgery in the future.After undergoing the treatment, an optimal static and functional occlusion was attained, yielding a favorable outcome.

Comparative evaluation of the dentoalveolar effects of three Class II correctors: A finite element analysis study.

To compare the stress distribution and total strain applied to the dentition, periodontal ligament (PDL) and cortical and trabecular bones by three Class II correctors using finite element analysis. Three-dimensional analysis of stresses and total strain of the dentition with three Class II correctors. Computational study. Three-dimensional finite element models of Class II elastics, the Forsus Fatigue Resistant Device (FRD) and the Carriere Motion Appliance (CMA) were constructed from a cone-beam computed tomography (CBTC) image of an orthodontic Class II patient. The distribution of stress (von Mises and principal stress) and the total strain (mm) in maxillo-mandibular dentition, PDL, cortical and trabecular bone were analysed. The highest von Mises yield and the maximum principal stress in the three models were found at the teeth, followed by the cortical bone, trabecular bone and PDL. The maximum stress and total deformation were located at the upper canines and lower molars in the Class II elastics and CMA models, in the upper first molars in the Forsus FRD and CMA, and in the lower first premolars in the Forsus FRD. In addition, stress was distributed in the anterior and posterior regions of the teeth, and the total deformation was found in the distal direction in the upper arch and in the mesial direction in the lower arch. The stress concentrations in the three models were located close to the active components of each appliance, producing specific patterns of stress distribution and displacement that should be taken into account when planning the type of appliance to be used for the correction of the Class II malocclusion.

Assessment of Changes in Craniofacial Structures, Bite Force and Periodontal Status Following Fixed Functional Appliance Therapy

The aim of this study was to determine the effect of Forsus Fatigue Resistant Device (FRD) appliance on craniofacial structures, bite force and periodontal status in Class II malocclusion patients. In this prospective interventional follow-up study, thirteen (13) Class II Division 1 patients in their post-adolescent age group with average age of 17.10 ± 1.63 year was treated with Forsus FRD. They were assessed for craniofacial changes, bite force and periodontal status at baseline, after alignment and leveling, after removal of FRD. Improvement in soft tissue profile was due to significant dentoalveolar changes. There were significant decreases in overjet, overbite, reference line to upper first molar, H angle (p<0.001) and significant increases in upper lip to E-line, reference line to lower molar and angular measurements like nasolabial angle, U1 to SN plane, Incisor Mandibular Plane Angle (p<0.001). The bite force was significantly decreased on the molar and the incisor region (p<0.001). A significant increase in plaque index (PI), gingival index (GI), probing pocket depth was noticed without any significant clinical attachment loss. Class II correction with Forsus FRD appliance was mainly due to significant dentoalveolar changes. Skeletal changes were non-significant. A decrease in the bite force was found with FRD. The magnitude of bite force was more in males compared with females. The increase in GI, PI, pocket probing depth implies the necessity of oral hygiene and plaque control measures. However, there was no significant change in clinical attachment level.

Assessment of the Treatment Outcomes of Skeletal Class III Malocclusion Using Forsus Fatigue Resistant Device Versus Intermaxillary Elastics

Purpose: This research aimed to evaluate the effect of treatment with Forsus Fatigue Resistant Device (FFRD) and traditional Class III elastics in treating class III malocclusion. Patients and Methods: The present study was applied to twenty female patients divided into two groups equally and randomly with age ranged from 14 to 16 years old, Group I treated with inverted FFRD which was attached to the mandibular first molar extra-oral tube and distal to the bracket of the maxillary canine and conventional class III elastics used in Group II. Results: Significant increase revealed in ANB angle in both groups while, there was significant decrease in S-A distance in FFRD group while, in relation to vertical skeletal measurement SN to mandibular plane angle (SN-MP) there was significant decrease in FFRD group and significant increase in intermaxillary elastic group. There was also significant proclination in maxillary anterior teeth and significant retroclination of the mandibular one. Conclusions: In class III malocclusion the use of inverted FFRD and elastics is successful treatment method to improve the maxilla-mandibular relationship by improving anteroposterior relationship (ANB angle). The FFRD is suggested in uncooperative patients as it did not require patient cooperation as class III elastics.

Dental and Skeletal Effects of Herbst Appliance, Forsus Fatigue Resistance Device, and Class II Elastics-A Systematic Review and Meta-Analysis.

Background: Our study aimed to systematically summarize the dentoskeletal effects of Herbst appliance; Forsus fatigue resistance device; and Class II elastics in adolescent Class II malocclusion. Methods: Five databases; unpublished literature; and reference lists were last searched in August 2022. Randomized clinical trials and observational studies of at least 10 Class II growing patients that assessed dentoskeletal effects through cephalometric/CBCT superimpositions were eligible. The included studies quality was assessed with the RoB 2 and ROBINS-I tools. A random-effects model meta-analysis was performed. Heterogeneity was explored with subgroup and sensitivity analyses. Results: Among nine studies (298 patients); two-to-three studies were included in each meta-analysis. Less post-treatment upper incisor retroclination (<2) and no overbite; overjet; SNA; SNB; and lower incisor inclination differences were found between Herbst/Forsus and Class II elastics. No differences in maxilla; condyle; glenoid fossa; and most mandibular changes were found between Herbst and Class II elastics; except for a greater 1.5 mm increase in mandibular length and right mandibular ramus height (1.6 mm) with Herbst. Conclusions: Herbst and Class II elastics corrected the molar relationship; but Herbst moved the lower molars more mesially. Apart from an additional mandibular length increase; no other dental and anteroposterior skeletal difference was found. Forsus was more effective in molar correction; overjet reduction; and upper incisor control than Class II elastics. Trial registration number OSF: 10.17605/OSF.IO/8TK3R.

Evaluation of effects of Forsus Fatigue Resistance device in correction of class II division 1 malocclusion in adolescent patient: A case report

Introduction: Class II malocclusion is one of the most prevalent malocclusions. The Class II malocclusions are caused due to forwardly placed maxilla, the backward position of the mandible, or a combination of both these factors. This disparity in skeletal base growth and position can be corrected during growth spurts using functional and fixed functional appliances. Description: An adolescent boy with Class II division 1 malocclusion, retrusive mandible, and increased overjet was treated with a pre-adjusted edgewise appliance (0.022-slot Gemini 3M -MBT prescription) along with a fixed functional appliance, Forsus TM Fatigue Resistant Device. The Skeletal age of the patient assessment using Hand wrist radiograph and CVM showed a major part of the adolescent growth spurt to be completed. Pre-treatment and post-functional cephalograms were traced and superimposed to compare changes in the skeletal base and dental structures. Result: The Class II molar and canine relationships were corrected to class I and the mandible showed forward positioning leading to correction for the skeletal base to class I. The facial profile showed marked improvement to an orthognathic pleasing profile. Conclusion: The purpose of this case report is to emphasize on use of fixed functional appliances in the treatment of adolescents with skeletal base discrepancies like Class II division 1 malocclusion. Intervention with fixed functional appliances at the appropriate skeletal age can prevent the need for extractions or other surgical procedures that may be needed to correct the malocclusion.

Effects of the Forsus fatigue-resistant device and mandibular anterior repositioning appliance in Class II malocclusion treatment

Fixed functional appliances have been used to treat Class II malocclusion by a huge number of professionals. This retrospective study aimed to analyze the effects of the Forsus fatigue-resistant device and compare the findings with a well-matched group treated with the mandibular anterior repositioning appliance (MARA). The Forsus group was composed of 14 patients at an initial mean age of 12.4 ± 1.3 years, treated with the Forsus fatigue-resistant device followed by a fixed orthodontic appliance. The MARA group comprised 18 patients at an initial mean age of 12.1 ± 1.3 years, treated with MARA followed by fixed orthodontic appliances. The untreated control group consisted of 14 patients matched with the other groups. Posttreatment changes were calculated as T1 - T2. Intergroup comparisons regarding treatment changes were performed using repeated-measures analysis of variance followed by Tukey's test. During treatment, the Forsus group showed a statistically significant decrease in maxillary protrusion and maxillomandibular sagittal discrepancy in the control group. The MARA group showed significantly greater retrusion of maxillary incisors than the Forsus and the control group. Overjet decreased significantly more in the treated groups in relation to the control group. Molar relationship improved significantly more in both treated groups, and both showed more correction than the untreated control group. The Forsus and MARA associated with fixed appliances effectively corrected the Class II malocclusion, mostly using dentoalveolar changes and maxillary growth restriction.

Objective: To determine treatment expectations of patients and parents, and the initial effects of fixed functional devices on oral health-related quality of life. Materials and Method: The study comprised 50 patients (39 female, 11 male, mean age 16.24 years) with Angle Class II,1 malocclusion, who were planned to be treated with forsus fatigue resistant device (FFRD). Treatment expectations of the patients and their parents/legal guardians were assessed before the treatment. The Oral Health Impact Profile (OHIP-14) was assessed 1-month after bonding (T1), and repeated 1-month after placement of FFRD (T2). Statistical analysis included Wilcoxon Signed Rank test for the evaluation of the differences in scores between treatment periods and Mann-Whitney U test for the evaluation of gender differences. Results: Main motivation for seeking orthodontic treatment was to improve dental appearance for the patients, and facial esthetics for their parents. Results declared that girls took their orthodontic problems more seriously than boys (p&amp;lt;0.05). The highest mean scores in OHIP-14 were achieved for “difficulty in eating”, and “feeling pain”. No significant difference between treatment periods was noted. Conclusion: This study highlighted the lack of serious adverse effects of the use of fixed functional devices on patients’ quality of life, and that patients might probably experience problems about physical status, mainly for functional limitations, rather than psychological status and social interactions. The findings may assist clinicians in understanding the concerns about these appliances.

CBCT evaluation of changes in condylar position following fixed functional appliance therapy: A comparative in vivo study

Objective: to evaluate and compare the condylar positional changes after the treatment of class ii malocclusion patients using powerscope appliance and forsus fatigue resistant device using pre-treatment and post-treatment CBCT images.Materials and Method: twenty subjects of adult age group having class ii division 1 malocclusion, horizontal to average growth pattern, orthognathic maxilla, retrognathic mandible, increased overjet were included in this study. The subjects were divided into two groups, forsus group and power scope group. A pre-treatment cbct scan was taken before the start of the treatment using Kodak 9000 3d extraoral imaging system. In group i, forsus fatigue resistant appliance was used to achieve class i occlusion. After completion of the forsus therapy another CBCT scan was taken. In group ii, powerscope appliance was used to achieve class i occlusion. Sequential activation of the powerscope appliance was done using crimpable shims depending on the amount of skeletal discrepancy in each patient’s mouth, until edgeto-edge bite was obtained. After completion of the powerscope appliance therapy, another CBCT scan was taken. After the pre and post treatment CBCT scans were analyzed, position of the condyle were assessed on the dicom images, using the cs 3d imaging software. The field of view was confined to the right and left TMJ. This was done so that condylar head, glenoid fossa and external auditory meatus were visualized and the data obtained were subjected to statistical analysis. Comparison of all the pre & post treatment parameters were done by paired-t test. Comparison of forsus and power scope group was done using independent t-test.Results: an average forward shift of mandibular condyle by. 85 mm was seen after 7- 9 months of treatment in forsus appliances therapy and. 79 mm after powerscope therapy indicating that the forward mandibular condylar shift is greater in forsus group than the powerscope group, the forward condylar shift of two groups was statistically insignificant with a mean difference of. 060 mm.Although both the forsus and powerscope groups showed a downward shift of mandibular condyle, there was a greater downward shift in forsus group (2.69 mm) compared to the powerscope group (2.29 mm), the downward condylar shift of two groups was statistically insignificant with a mean difference of 0.42mm.Conclusion: forsus appliance brought about a more anterior and downward shift in the mandibular condylar position, compared to power scope group but the difference were not statistically significant between the two appliances i.e. Forsus and powerscope.

Correction of Class II malocclusion in a patient using the forsus fatigue resistant appliance – A case report

Functional appliances, fixed or removable, are primarily orthopaedic tools used to influence the facial skeleton in a growing child. Class II malocclusion with a retrognathic mandible in a non -compliant post pubertal patient can be treated using the fixed functional appliances. One such appliance which is in common use in clinical practice today is the Forsus Fatigue Resistant Device. To treat a 13 year old female patient having a class II base, a retrognathic mandible, horizontal growth pattern, convex profile, an overjet of 7mm, mild proclination of maxillary and mandibular incisors, Class II molar and canine relationship bilaterally and a positive VTO.

Evaluation of displacements and stress changes in the maxillo-mandibular complex with fixed functional appliance skeletally anchored on mandible using miniplates: A finite element study.

OBJECTIVES:Fixed functional appliances (FFA) have been used for correction class II malocclusion but cause proclination of teeth. Recently, few studies have advocated the use of skeletal anchorage along with FFAs to prevent this side effect. The purpose of the study was to evaluate the stress distribution and displacements produced by miniplate anchored Forsus-fatigue resistant device on the maxilla, mandible, and the dentition in a Class II malocclusion patient by finite element analysis.MATERIALS AND METHODS:Cone beam computed tomography of a class II division 1 patient indicated for FFA was used to construct a 3D finite element model using MIMICS 8.11 and Hypermesh 13.0 software. Various material properties, boundary, and loading conditions were then applied. The model was analyzed for principal stress and von Mises stress in maxilla, mandible, and their dentition using ANSYS 12.1 software.RESULTS:In the mandible, maximum principal and von Mises stresses were seen in the cortical bone area in the symphyseal region, whereas mandibular teeth showed comparatively lesser stresses. In the maxilla, higher principal and von Mises stresses were seen in the maxillary molar region compared to the maxillary cortical bone. The entire mandible was displaced antero-inferiorly, whereas the maxilla showed a postero-superior displacement.CONCLUSION:Using skeletally anchored FFA results in more stresses and displacements in the skeletal structures compared to the dentition.

Combination of forsus fatigue resistant device and premolar extraction for the management of a skeletal class II malocclusion

Treatment of class II malocclusion requires accurate diagnosis and treatment planning. This case report outlines the successful management of a growing skeletal class II using Forsus fatigue resistant device for correction of skeletal parameters and premolar extractions for correction of dental parameters. The patient’s profile improved significantly with a 4° reduction in ANB angle. An ideal overjet, overbite and molar relation were also attained.

Improvement in Soft Tissue Profile &amp; Upper Airway Dimensions Following Forsus FRD – A Prospective Clinical Study

Aim: To assess improvement of soft tissue profile and upper airway dimensions in skeletal Class II adolescent patients treated with Forsus Fatigue Resistant Device (FRD). Material and Methods: This prospective observational study was planned with a sample size of 15 subjects (5 male &amp; 10 female) diagnosed with mandibular hypoplasia as a cause of Class II malocclusion, requiring fixed functional therapy, with the inclusion criteria of age of 16-25 yrs with the ANB value range of 4°+2° with a non-extraction treatment plan. Pre-functional records were recorded once the leveling and alignment phase was complete and Forsus FRD attachments were placed. Treatment continued till the desired objectives were achieved. Post functional records were made at the end of functional therapy, after removal of Forsus FRD. Along with soft tissue analysis on lateral cephalogram, Acoustic Pharyngometry (AP) was recorded for each patient (Eccovision®” (sleep group solutions Florida, US) for airway changes before and after Forsus FRD. Results: Post functional treatment indicated statistically significant changes in nasolabial angle, upper lip position, upper lip strain, H angle and mento-labial sulcus. On evaluation of upper airway dimensions of the subjects by pharyngometry , there were statistically significant changes in mean upper airway volume and area. Conclusion: Study concluded that Forsus FRD is an effective method for correction of skeletal Class II malocclusion among adolescent patients, enhancing marked improvement in soft tissue profile of the patients, along with significant increase in volume and area of hypopharyngeal and oropharyngeal spaces, thus improving the upper airway patency.
